Beyond Compliance: How MSHA Leaders Drive Hazard Awareness, Electrical Safety & Guarding Excellence

After years of working on various mine sites with major players like Rio Tinto and Newmont, I’ve learned one truth that never changes: Significant Injuries and Fatalities (SIFs) don’t happen because people don’t care. They happen when training, planning, communication and controls break down, especially when leadership is not engaging consistently with the front line.

Mining is unforgiving. The pace is fast, the equipment is massive and the work is complex. But when leaders commit to consistent training, clear communication, and disciplined pre‑task planning, they create an environment where miners can recognize hazards early, apply the right controls, and go home safely every single day.

Below are three core areas - Hazard Recognition, Machine Guarding and Electrical Safety - that MSHA leaders must champion relentlessly to prevent SIFs and build a culture where people look out for each other.

Hazard Recognition: The Foundation of SIF Prevention

Every SIF prevention strategy begins with one simple habit: Daily pre‑task hazard recognition. Mines change constantly - weather, ground conditions, equipment movement, production pressures, contractor activity and even small layout changes can introduce new risks. When crews skip or rush pre‑task plans, they lose the opportunity to identify hazards before they become incidents.

Effective hazard recognition requires more than checking a box. It demands:

  • Detailed and consistent training for all hazards workers encounter
  • Clear expectations for identifying hazards and proper controls for risk reduction
  • Active and effective communication between supervisors and operators
  • A structured pre‑task risk assessment process that guides the crews
  • Use of the hierarchy of controls to reduce risk and allow workers to fail safely

The hierarchy of controls is especially important. Too often, teams default to PPE or administrative controls because they’re quick. But preventing SIFs requires engineering controls, isolation, redesign or elimination whenever possible. Leaders must reinforce that the goal is not simply to “complete the form”- it’s to reduce risk.

When miners understand why hazard recognition matter and when leaders consistently engage with them,  pre‑task planning becomes a powerful tool for preventing life‑altering injuries or fatalities.

Machine Guarding: Protecting Workers From the Equipment That Drives the Mine

Mining equipment is powerful, efficient and unforgiving. Conveyors, crushers, screens, haul trucks, and mobile equipment all present caught‑in, struck‑by, and pinch‑point hazards that can lead to catastrophic outcomes. Machine guarding is one of the most effective SIF prevention controls we have, yet it is commonly cited, overlooked and continues to cause significant injuries and fatalities every year.

Proper machine guarding requires:

  • Daily inspections of guards, panels and access points
  • Immediate maintenance when guards are damaged, missing or bypassed
  • Clear communication when equipment is unsafe to operate
  • Documented corrective actions to ensure issues don’t repeat

A guard that is “almost in place” or “temporarily removed” is a tragedy waiting to happen. Leaders must set the expectation that equipment with no or improper guarding must be locked out until corrected. No exceptions, no shortcuts, no “just for a minute.”

Machine guarding is also a cultural issue. When miners see leaders walking past missing guards without addressing them, they assume it’s acceptable. But when leaders stop, engage, and correct the issue, they send a stronger message: We protect each other. We don’t gamble with SIF potential.

Electrical Safety: Making an Unpredictable Hazard More Predictable

Electricity is silent, invisible and deadly. Whether it’s a high‑voltage system, a portable tool, a pump, or a piece of mobile equipment, electrical hazards demand disciplined inspection, maintenance, and isolation procedures.

The most effective electrical safety programs include:

  • Routine inspections of cords, panels, disconnects and grounding
  • Preventive maintenance to identify wear before it becomes failure
  • Strict lockout/tagout (LOTO) procedures - no assumptions, no shortcuts
  • Clear documentation of isolation steps and verification
  • Training that reinforces real‑world scenarios, not just regulatory requirements

Electrical accidents are in the top 10 most common causes of injuries on mine sites, and are the 6th most common cause of fatal injuries. Mining has an electrical fatality rate 8 to 12 times higher than all other U.S. industries combined.

Isolation and LOTO is one of the strongest controls in the hierarchy, but it only works when applied effectively and consistently. Many electrical SIFs occur because someone believed equipment was de‑energized when it wasn’t.

Leaders must reinforce that verification is non‑negotiable. When energy sources are isolated, tested, locked and tagged (with every individual involved on their own personal lock and key), there is no uncertainty; there is predictability. If miners feel rushed or pressured, they may take risks. Strong leadership removes that pressure and replaces it with clarity: We will never sacrifice safety for speed.

Training, Communication and Documentation: The Glue That Holds Safety Together

Hazard recognition, machine guarding and electrical safety all rely on three pillars:

  • Training that is practical, hands‑on and relevant to the work
  • Communication that is clear, consistent and two‑way
  • Documentation that reinforces accountability and learning

Training should go beyond compliance. Miners need to understand why controls matter, how SIFs occur and what early warning signs look like. Communication must be constant. Pre‑shift meetings, field interactions, job walk‑downs and post‑task reviews all help reinforce expectations. Documentation ensures issues are tracked, corrected and prevented from recurring.

When these three elements work together, they create a "system" that is proactive rather than reactive.

Leadership in the Field: The Most Powerful SIF Prevention Tool

The strongest safety cultures are built by leaders who show up - not just in the office, but in the field. When senior leaders and supervisors walk the site, talk with miners, ask questions and participate in pre‑task plans, they demonstrate genuine commitment.

Field engagement allows leaders to:

  • See hazards firsthand
  • Understand the challenges miners face
  • Reinforce proper controls
  • Build trust and credibility
  • Encourage workers to look out for each other

Most importantly, it reminds miners that safety isn’t about rules - it’s about people. Every worker has someone waiting for them at home. When leaders connect safety to families, values and teamwork, miners become more invested in protecting themselves and each other.

Final Thoughts

Preventing SIFs can be a reality with discipline, leadership and commitment. It all starts with comprehensive systems, proper training, effective communication and detailed documentation.
